At the food bank, we serve low income families and individuals in need of emergency food assistance. Households can access the food bank once a month for a 3 to 5 day emergency food supply once per month or as needed based on the action plan developed with a case manager. Additionally, they can come in once per week for bread products when they are available.

How Can I Apply?

To qualify for this program:

• You must be a Davis or Morgan county resident (you will need to bring in a current utility bill or lease agreement to show proof)
• You must meet up to 150% of the Federal Poverty guidelines (see chart below)

If you qualify for the program, you will need to fill out an application, a client budget worksheet, and a needs assessment, and turn it in to the Layton Office. Once you turn in your application, you will meet with a case manager.  You should also bring in all of the following paperwork in order to get approved for emergency food:

• A current utility bill or lease agreement to show proof of residency is Davis or Morgan county
• Social Security Cards (no copies) for all members of the family--if you don't have original social securty cards, all of the following are accepted: birth certificate, school records, Department of Workforce Services printout, and IRS numbers
• Proof of income (current paycheck stubs, award letter from DWS, SSI, SSD, pension, child support, etc.)
• Photo identification for all adult members in household

All adults 18 or older must apply for their own food card unless they are a spouse or a child still in high school.
