The mobile food program assists the elderly and disabled who are unable to come to the food bank themselves to receive an emergency food supply. Once per month, the food bank offers a 3 to 5 day emergency food supply that is delivered to the home. The mobile food program is limited to the elderly or disabled who have no transportation due to a medical condition.

How Can I Apply?
To qualify for this program:

• You must be a Davis or Morgan county resident (you will need to bring in a current utility bill or lease agreement to show proof)
• You must be elderly (over 65) and/or disabled (and unable to leave the home) OR you must have a disabled spouse/child/relative living with you who requires your constant attention
• You must meet up to 150% of the Federal Poverty guidelines (see chart below)

If you qualify for the program, you will need to fill out an application, a client budget worksheet, and a needs assessment, and turn it in to the Layton Office. Once you turn in your application, you will meet with a case manager.  You should also bring in all of the following paperwork in order to get approved for emergency food:

• A current utility bill or lease agreement to show proof of residency is Davis or Morgan county
• Social Security Cards (no copies) for all members of the family--if you don't have original social securty cards, all of the following are accepted: birth certificate, school records, Department of Workforce Services printout, and IRS numbers
• Proof of income (current paycheck stubs, award letter from DWS, SSI, SSD, pension, child support, etc.)
• Photo identification for all adult members in household
